Fees are due at the time of service, payable by cash or check. If you are using a managed care plan and I am an in-network provider, you are responsible for your deductible and co-pay. Mamta Daswani, who handles my insurance billing, will bill your insurance provider and send you an invoice for the parts of your bill that you are responsible for. We can also provide a statement if you have a medical savings account through your employer.

If I am an out-of-network provider, you are responsible for your bill with me, and the insurance company is responsible for reimbursing you. Mamta can bill your insurance company or we can provide you with a statement for you to submit the claim yourself.

I have a 24-hour cancellation policy. You will be charged for sessions cancelled without 24 hours notice. These fees are not covered by insurance.

I also charge a $35 fee for checks that are returned for insufficient funds.
